What is the USPS Postal Service 1583 form?

The U.S. Postal Service requires all Commercial Mail Receiving Agencies (CMRA), like OfficeQ Miami, to collect a notarized USPS 1583 form from every customer that opens an account with us. This form allows mail to be opened, scanned, or shipped after we've received it at one of our address.

It ensures that you, and only authorized parties, are able to access mail that is addresses to you.

The 1583 Form authorizes us to accept mail in your name. It does not change your address with the USPS or tell us where to forward your mail.

Due to USPS regulations, you need to have the form notarized before sending it to us. The notary's seal proves that you are the person who is authorizing OfficeQ Miami to receive mail in your name or your company's name.

Your personal information on the USPS 1583 Form will never be public. This information is only for the USPS Form 1583.

You can e-mail it to info@officeqmiami.com or send by mail to 13550 SW 88th Street Suite 220, Miami, Fl 33186.
